| Suppliers: |

- For all potential suppliers please complete the Suppliers Application Form and send via e-mail or fax. Detailed information in regards to your products and company is welcomed.
- For overseas supplier please include any certifications or registrations that are available. (ex. FDA, UL, Ce)
- Detailed product warranty information is required which be directly faxed or e-mailed to Titan Chair LLC.

| Distributors/Dealers/Retailers: |

- Please fill out a Distributors Applications Form provided below. In order to process the application in a fast manner all information must be detailed and accurate.
- There is no minimum shipping order. All Shipping Charges are not included in the quotations and be requested at time of purchase. Shipping options are available through UPS, FedEx and R+L Carriers. Shipping rates are dependant on size, weight, volume and form of transport. Typically the larger orders have a better shipping rate.
- Exchange or returns are available dependant on the condition of items and time of purchase.
- Cash, Credit and wire transfers are accepted forms of payment.
- Distributor's application form must be completed and sent.
- Only select items will be available to wholesalers, Distributors and Retailers.
